Register with our practice

The doctors welcome new patients who live within our practice area.  Please call in to our reception to pick up a registration form or download below.

The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor.  This information helps the NHS to quickly trace your medical records and send them to your new surgery.

Once you have completed and signed the forms bring them along to the surgery Monday to Friday to be registered with the practice. You will be required to produce proof of address and this can be a recent utility or council tax bill to enable us to complete your registration.

You will then be given an appointment for a New Patient Health Check and asked to arrive 15 minutes early to complete a New Patient Questionnaire.  The new patient questionnaire will ask you some basic health/lifestyle questions, so we can provide the best service possible from the start.  We offer all our new patients a medical check with our Practice Nurse or Health Care Assistant. This will consist of a short medical examination e.g height, weight, blood pressure, urine testing, medication review and the nurse takes the opportunity to ask about previous medical history.

Named GP

From 1st April 2015, under our NHS contract we were required to allocate a named accountable GP to ALL patients. Your named accountable GP will be responsible for your overall care. We have allocated a named accountable GP to existing patients and also to all newly registered patients.  If you have not already been informed of who your named accountable GP is and would like to know, please ask at reception.

If you are aged over 75 years and were registered prior to 1st April 2015 you will have already been allocated a named accountable GP and we will have written to you to inform you of this. If you are new to the Practice and are aged 75 years and over, your registration pack will detail who your named accountable GP is.

Please remember that you can see the GP of your choice at the Practice and do not have to see your named accountable GP, they just have overall responsibility for your care. Please continue to book your appointments with the GP of your choice.
