Register with our practice
Glovers Lane welcomes new patient. Registering to be a patient at our practice is simple, first check that your address is in our practice area in the map below then fill in our online registration form found below and we will take care of the rest. You will need the details of your current GP practice so we can obtain you medical records. The form will ask for your NHS number but this is not essential.
Once you have registered we may ask patients who are not housebound to attend for a New Patient Health Check. This is an important part of the registration process and helps us ensure we are providing the best level of care for any conditions you may have.
If you are unable to use our online registration form or would prefer to fill in a paper form or need help filling form in please attend the surgery an we will help with any problems you might have.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.Named GP
From 1st April 2015, under our NHS contract we were required to allocate a named accountable GP to ALL patients. Your named accountable GP will be responsible for your overall care. We have allocated a named accountable GP to existing patients and also to all newly registered patients. If you have not already been informed of who your named accountable GP is and would like to know, please ask at reception.
If you are aged over 75 years and were registered prior to 1st April 2015 you will have already been allocated a named accountable GP and we will have written to you to inform you of this. If you are new to the Practice and are aged 75 years and over, your registration pack will detail who your named accountable GP is.
Please remember that you can see the GP of your choice at the Practice and do not have to see your named accountable GP, they just have overall responsibility for your care. Please continue to book your appointments with the GP of your choice.
