Sick Notes for coronavirus

If you need a sick note to give to your employer please make an appointment to speak to one of our gp’s. If an extension of sick note is required please write down the dates needed for cover and hand into the surgery or fill in our online administration form and this will be added to a gp list for consideration.  Sick notes approved by the doctor will be sent to you mobile or can be printed off for collection.
